If you are sending a DEPOSIT ONLY, submit a copy of the ENTRY FORM with the DEPOSIT PAYMENT to ensure it is applied to the correct team.

**  Please make sure to submit all paperwork no later than the ENTRY DEADLINE so entries can be processed in a timely manner. **

The NW Indiana event will also require a CONTACT SHEET - This will be sent via email to coaches

 much closer to the event & once filled out online, NW Indiana will receive a direct copy.  No need to fill out and email.
